Guidance on completing the form

This 360-degree assessment and the quality of the personal report you will receive, will depend on who you nominate to provide you with feedback. Your report is personal and will not be shared with anyone, though we do encourage you to share as you wish with those involved in your professional development.  

Don’t simply select those you like and/or like you. Choose individuals representing the mix of working relationships - up, down and across the organisation and/or those organisations with whom you are collaborating in a project or similar venture.

You should include in addition to your own information as self:

  • Your line manager.
  • Select four peers; these are individuals working at a similar level to you who have seen you in a number of work situations.
  • Select four team members; these are individuals you lead/manage.
  • Select four stakeholders; individuals who may be key customers (internal or external), suppliers, contractors who you interface with on a reasonably regular basis.

Enter your own information and the names of your 13 ‘feedbackers’, making clear the different working relationships: line manager, peers, team members, and stakeholders. We also require your feedbackers’ internal/external email addresses.
